Ureteric reconstruction is a complex surgical procedure and just like any major surgery, it carries certain risks and potential complications. These complications can range from minor postoperative discomfort to more severe long-term issues requiring additional intervention.

Below are the main risks associated with the procedure:

1. Stricture Recurrence

One of the major risks after ureteric reconstruction is the recurrence of ureteral strictures (narrowing of the ureter due to scarring).

Scar tissue may form at the surgical site, leading to partial or complete obstruction, which can impair urine flow and cause hydronephrosis (swelling of the kidney due to urine buildup).

Recurrence is more likely in patients with a history of prior surgeries, radiation therapy, infections, or chronic inflammation.

If a stricture develops, further interventions such as balloon dilation, stent placement, or revision surgery may be required.

2. Urinary Leakage and Urinoma Formation

If the surgical connection between the ureteral ends (anastomosis) is not completely watertight, urine can leak into surrounding tissues.

This leakage can lead to the formation of a urinoma (a localised collection of urine outside the urinary tract), which may cause pain, swelling, or infection.

Management:

Small leaks may resolve with temporary stenting of the ureter.

Larger leaks may require drain placement or surgical revision.

3. Postoperative Infections

Surgical wounds and the urinary tract are susceptible to infections, especially in patients with a history of recurrent urinary tract infections (UTIs) or those who required urinary diversion before surgery.

Symptoms of infection may include fever, pain, redness, increased urination, and foul-smelling urine.

Prevention & Management:

Prophylactic antibiotics are given before and after surgery to reduce infection risks.

Maintaining hydration and ensuring proper urine drainage can help prevent bacterial growth.

4. Formation of Urinary Fistulas

In rare cases, an abnormal connection (fistula) may form between the ureter and nearby organs, such as the intestines, bladder, or skin.

This can cause continuous urine leakage, leading to skin irritation, infection, and discomfort.

Treatment:

Small fistulas may heal on their own with prolonged catheter drainage.

Larger fistulas may require surgical correction.

5. Metabolic Abnormalities (If an Ileal Segment is Used)

When a segment of the intestine (such as the ileum) is used to replace a damaged ureter, metabolic complications can arise due to absorption of urinary solutes by the intestinal lining.

Possible metabolic issues include:

Metabolic acidosis (excess acid buildup in the blood due to bicarbonate loss).

Electrolyte imbalances (loss of sodium, potassium, chloride).

Increased mucus production, which may lead to blockage of urine flow.

Management:

Dietary modifications, oral bicarbonate supplements, and careful electrolyte monitoring are necessary for patients with ileal ureter substitution.

6. Bleeding (Haemorrhage)

Bleeding can occur during surgery (intraoperative bleeding) or after surgery (postoperative haemorrhage).

High-risk factors include:

Patients with prior radiation therapy (which weakens blood vessels).

Extensive fibrosis or scarring.

Use of blood-thinning medications (such as aspirin or anticoagulants).

Treatment:

Minor bleeding may resolve with blood transfusion or supportive care.

Severe bleeding may require a second surgery to control the haemorrhage.

7. Kidney Function Decline

If urine flow is not properly restored after reconstruction, backpressure on the kidney can lead to reduced kidney function over time.

Patients with pre-existing kidney disease are at higher risk of chronic kidney impairment.

Regular follow-ups with kidney function tests (creatinine, eGFR) and imaging (ultrasound, CT scan) help monitor kidney health.

8. Ureteral Stent-Related Issues

Ureteral stents are often placed during surgery to keep the ureter open during healing, but they can sometimes cause:

Pain and discomfort in the bladder or kidney.

Frequent urination or hematuria (blood in urine).

Stent migration (movement of the stent from its proper position).

Most stents are removed within 2-6 weeks (about 1 and a half months) after surgery to prevent complications.
