Treatment for MDS depends on the disease’s risk category, patient age, overall health, and specific genetic changes. Broadly, therapy aims to:

· Improve blood cell production

· Reduce the need for transfusions

· Prevent progression to acute myeloid leukemia (AML)

· Control symptoms and maintain quality of life

1. Supportive Care

Supportive therapy addresses the complications of low blood counts.

· Blood transfusions — Red blood cell transfusions relieve anemia; platelet transfusions reduce bleeding risks.

· Growth factors — Erythropoiesis-stimulating agents (ESAs) like epoetin alfa encourage red blood cell production. Granulocyte-colony stimulating factor (G-CSF) boosts white blood cells to prevent infections.

· Iron chelation therapy — Frequent transfusions can cause iron overload, which is managed with chelating agents like deferasirox.

2. Disease-Modifying Drugs

These medicines target abnormal bone marrow cell behavior and help delay disease progression.

· Hypomethylating agents (HMAs) — Azacitidine and decitabine are standard drugs that modify DNA methylation patterns, allowing normal genes to function better. They are often given in cycles and can improve survival.

· Immunomodulatory drugs — Lenalidomide is effective for patients with deletion of chromosome 5q (del(5q) MDS), often reducing transfusion dependence.

3. Intensive Therapies

For high-risk patients, more aggressive approaches may be needed.

· Chemotherapy — Similar to AML treatment, it is used when MDS transforms into acute leukemia.

· Allogeneic stem cell transplantation — The only potential cure for MDS, it replaces diseased bone marrow with healthy donor cells. Suitable mainly for younger or medically fit patients due to its risks.

4. Targeted and Emerging Therapies

New drugs are under research to improve MDS outcomes:

· Luspatercept — Helps improve anemia in patients unresponsive to ESAs.

· APR-246 — Targets TP53-mutated MDS.

· Venetoclax combinations — Showing promise when paired with HMAs in high-risk disease.

Treatment Planning According to Risk Level

· Low-risk MDS — Emphasis on supportive care, growth factors, and lenalidomide for del(5q) patients.

· Intermediate-risk MDS — Hypomethylating agents are often started early to slow progression.

· High-risk MDS — Aggressive treatment, including HMAs, intensive chemotherapy, or stem cell transplantation to prevent AML transformation.
