A 55-year-old male with a history of hypertension and progressive shortness of breath for 5 years was diagnosed with a large left ventricular aneurysm.

Diagnostics:

Echocardiography revealed a left ventricular dilatation with a large thrombus. Coronary Angiogram confirmed complete blockage of all three major coronary arteries as well.

Treatment Plan and Surgery:

An urgent surgery was planned to repair the left ventricular aneurysm, remove the underlying thrombus, and perform coronary artery bypass grafting. On November 12, 2024, the patient underwent an 8-hour surgery using cardiopulmonary bypass for cardiac protection. A prosthetic graft was used for the repair, with careful management to minimize blood loss and prevent complications.

Challenges:

The large aneurysm and intramural clot posed significant surgical risks. Multidisciplinary preoperative optimization and intraoperative precision were critical to mitigate the high mortality risk associated with left ventricular aneurysm repair.

Recovery and Outcome:

Postoperative care involved comprehensive management by intensive care, anaesthesia, nursing, and physiotherapy teams. The patient recovered well, regaining mobility, and was discharged 9 days after surgery, with a total hospital stay of 14 days.

The prompt diagnosis and multidisciplinary approach enabled the successful resolution of a potentially fatal left ventricular aneurysm and coronary artery disease.

By addressing both structural and coronary complications, the team achieved a favourable outcome, demonstrating the importance of integrated care in managing
