Overview

Many women experience irregular periods, acne, weight changes, or difficulty conceiving and often wonder whether they have PCOD or PCOS. These terms are frequently used interchangeably, which can create confusion about their meaning, symptoms, and treatment.

Although PCOD (Polycystic Ovarian Disease) is a commonly used term, PCOS (Polycystic Ovary Syndrome) is the medically recognised condition. PCOS is a complex hormonal and metabolic disorder that occurs due to changes in ovulation, androgen levels, and insulin sensitivity. The severity and symptoms may differ greatly from one woman to another.

What is PCOD?

PCOD stands for Polycystic Ovarian Disease. It is a term commonly used to describe changes in the ovaries where multiple immature follicles may develop because of irregular ovulation.

These follicles are not true cysts but are small fluid-filled sacs containing immature eggs. When ovulation does not occur regularly, these follicles may remain in the ovaries and appear as multiple small follicles on ultrasound.

PCOD is often associated with irregular menstrual cycles, hormone changes, difficulty with ovulation, and acne or mild hair changes in some women

What is PCOS?

PCOS stands for Polycystic Ovary Syndrome. It is a hormonal and metabolic disorder that affects ovarian function and hormone control. Women with PCOS may experience:

Irregular or absent ovulation
Increased levels of androgens (male hormones) in some cases
Irregular periods
Difficulty getting pregnant
Acne or excess facial/body hair
Metabolic changes such as insulin resistance

Not every woman with PCOS will have ovarian cysts visible on ultrasound, and not everyone with ovarian cysts has PCOS.

PCOD vs PCOS: What Is the Difference?

Definition:

PCOD- Ovaries release immature or partially mature eggs that accumulate as cysts over time.

PCOS- A more complex hormonal and metabolic disorder affecting the body more broadly.

Ovarian Changes:

PCOD: Ovaries often become enlarged; eggs are usually still produced.

PCOS: Ovulation is frequently disrupted due to hormonal imbalance.

Severity:

PCOD: Generally considered a milder condition.

PCOS: More clinically significant and complex.

Fertility Impact:

PCOD: Fertility is less commonly affected.

PCOS: More strongly linked to infertility.

Metabolic & Long-Term Health Risks:

PCOD: Not typically associated with major metabolic complications.

PCOS: Often involves insulin resistance, weight gain, and a higher long-term risk of type 2 diabetes.

Causes:

PCOD: Closely tied to lifestyle factors like diet and stress.

PCOS: Rooted in broader hormonal and metabolic dysfunction.

Management/Treatment:

PCOD: Often improves with lifestyle changes alone.

PCOS: Frequently requires medical or hormonal treatment alongside lifestyle changes

What Causes PCOD and PCOS?

The exact cause of PCOS is not fully understood. It is believed to develop due to a combination of hereditary, hormonal, as well as environmental factors.

Possible contributing factors include:

Genetics: Women with a family history of PCOS, hormonal disorders, or type 2 diabetes may have a higher risk.
Hormonal Imbalance: Changes in reproductive hormones can affect ovulation and menstrual regularity.
Insulin Resistance: Some women with PCOS have insulin resistance, where the body does not react properly to insulin. This may cause elevated insulin levels and may cause increased androgen production.
Lifestyle Factors: An unhealthy diet, lack of physical activity, poor sleep patterns, and stress may worsen symptoms in some individuals.

Symptoms of PCOD and PCOS: What Should You Watch For?

Symptoms can overlap between PCOD and PCOS, although their severity and presentation can vary from person to person. Common symptoms include:

Menstrual Changes

Irregular periods – Cycles that don't follow a predictable pattern, varying widely in length from month to month. This happens because ovulation isn't occurring regularly.
Delayed periods – Periods that consistently arrive later than expected, often due to hormonal imbalances disrupting the normal cycle timing.
Heavy or prolonged menstrual bleeding – When the uterine lining builds up longer than usual (due to infrequent ovulation), it can result in heavier flow or bleeding that lasts more days than normal.
Missed periods – Some cycles may be skipped entirely, a sign that ovulation didn't happen that month at all.

Hormonal Symptoms

Excess facial or body hair growth (hirsutism) – Elevated androgen (male hormone) levels can cause coarse hair to grow on the face, chest, or back in a male-pattern distribution.
Acne – Higher androgen levels increase oil production in the skin, which clogs pores and leads to persistent breakouts, often along the jawline and chin.
Oily skin – Similar to acne, excess androgens stimulate the sebaceous glands, giving skin a greasier texture than usual.
Hair thinning or scalp hair loss – Androgen excess can also cause thinning at the crown or a receding hairline, a pattern sometimes called female-pattern hair loss.

Metabolic Changes

Difficulty controlling weight – Hormonal imbalance and insulin resistance make weight gain easier and weight loss harder, even with consistent effort.
Increased abdominal fat in some individuals – Fat tends to accumulate more around the midsection, which is linked to how insulin resistance affects fat storage.
Dark patches of skin (acanthosis nigricans) – Velvety, darkened skin around the neck, underarms, or groin often signals underlying insulin resistance, a common feature in PCOS.

Fertility Concerns

Difficulty becoming pregnant – Irregular or absent ovulation means an egg isn't reliably released each cycle, which can make conception harder without medical support.

How Are PCOD and PCOS Diagnosed?

Diagnosis entails a detailed evaluation of symptoms, medical history, physical examination, and specific investigations.

1. Blood Tests

Blood tests may help evaluate androgen hormone levels, blood glucose levels, insulin resistance, cholesterol levels, thyroid hormones, and other hormonal conditions that may cause similar symptoms.

2. Pelvic Ultrasound

A pelvic ultrasound helps assess ovarian appearance, presence of multiple small follicles, and thickness of the uterine lining. However, ultrasound findings alone cannot confirm PCOS.

3. Rotterdam Criteria for PCOS Diagnosis

Doctors usually diagnose PCOS when at least two of the following three features are present after excluding other causes:

Irregular or absent ovulation
Signs of increased androgen levels (clinically or through blood tests)
Polycystic ovarian appearance on ultrasound

Treatment Strategies for PCOD and PCOS

Treatment depends on symptoms, age, overall health, and whether pregnancy is desired. The goal is to regulate menstrual cycles, manage symptoms, improve fertility when required, and reduce long-term health risks.

Treatment for PCOS

Polycystic Ovary Syndrome is managed through a combination of lifestyle, medical, and long-term monitoring strategies, since it is a systemic endocrine and metabolic disorder. Lifestyle modification is the foundation of treatment, with even a 5–10% reduction in body weight significantly improving hormonal balance and symptoms.
This is supported by a low-glycemic, anti-inflammatory diet and a structured exercise routine combining cardio and strength training. Since insulin resistance is central to PCOS, doctors often prescribe Metformin to improve insulin sensitivity and reduce the risk of type 2 diabetes, alongside regular blood sugar monitoring.

Treatment for PCOD

Polycystic Ovarian Disease is a comparatively milder condition and generally responds well to lifestyle changes alone, without the need for extensive medical intervention. The first-line approach involves weight management through a balanced, low-glycemic, high-fiber diet, combined with regular moderate exercise for about 30–45 minutes.
Dietary adjustments play a major role, focusing on reducing refined sugar and processed carbohydrates while increasing protein, healthy fats, and anti-inflammatory foods such as leafy greens, nuts, and omega-3 sources.
