Children grow and develop in a fairly predictable sequence: first head control, then sitting, crawling, walking, and words, each building on the one before. Most kids hit these milestones within a broad age window rather than on an exact date, and normal variation is common. See a doctor if a child loses a skill they already had, misses several milestones in a row, or shows no progress at all by the outer edge of the expected range.

Why does this question come up so often?

Every parent ends up comparing their child to the neighbour's kid, a cousin, or whatever the pamphlet from the last check-up said should be happening by now. It's an instinct, and mostly it leads nowhere useful, since children genuinely don't all follow the same pace.

That said, growth and development tracking exists for a reason. It's one of the more reliable ways paediatricians catch something worth addressing early, whether that's a nutritional gap, a hearing issue affecting speech, or a developmental delay that responds far better to early intervention than to a wait-and-see approach three years later.

This isn't a diagnostic checklist, and it won't replace an actual paediatric assessment. What it can do is give you a realistic sense of what's typical, what's still within normal range, and what's actually worth flagging.

Growth measurements vs developmental milestones

Growth measurements are physical, height, weight, and head circumference, plotted on a standardised chart against thousands of other children the same age. A child doesn't need to sit at the 50th percentile to be healthy; what matters more is a consistent growth curve over time, rather than one number on one visit.

Developmental milestones are functional, which means what a child can do. These fall into a few broad categories:

Gross motor skills, like sitting, crawling, walking, running.
Fine motor skills, like grasping, stacking blocks, and holding a pencil.
Speech and language, from babbling to full sentences.
Social and emotional skills, like eye contact, sharing, and recognising familiar faces.
Cognitive development, problem-solving, memory, understanding cause and effect.

A child can track perfectly on the height and weight chart while lagging in speech, or vice versa. Paediatricians assess both separately, since one doesn't reliably predict the other.

Milestones by age group

By 3 months: lifts head during tummy time, follows objects with the eyes, begins social smiling, makes cooing sounds.
By 6 months: rolls over, sits with support, reaches for objects, responds to their name, begins babbling.
By 9 months: sits without support, crawls or scoots, transfers objects between hands, says simple sounds like "mama" or "baba" without clear meaning yet.
By 12 months: may take first steps, says one or two clear words, points at objects, waves bye-bye, understands simple instructions.
By 18 months: walks independently, says 10 to 20 words, follows two-step instructions, shows early signs of independence and mild tantrums.
By 2 years: runs, climbs, combines two words into short phrases, plays alongside other children, begins pretend play.
By 3 years: speaks in short sentences, understands basic concepts like big/small, uses stairs alternating feet, engages in simple pretend play with others.

These ranges aren't rigid deadlines. A child walking at 11 months and another at 15 months are both entirely normal. What matters more is the general trajectory, not a single missed date.

For babies born early, paediatricians usually track milestones against adjusted age, the age counted from the original due date rather than the actual birth date, for at least the first two years.

A child with a strong family history of "late talkers" or "late walkers" may also simply be following a normal, if slower, personal pattern. Prior medical history, chronic illness, or hospitalisation can shift timelines too, and a paediatrician factors all of this in rather than applying one fixed chart to every child.

Why does each stage matter?

Early milestones aren't just checkboxes; they build on each other. Head control supports sitting. Sitting supports the balance needed for crawling and walking. Babbling lays the groundwork for words, and words for sentences. A significant delay at one stage can, left unaddressed, make the next stage harder to reach on schedule. This is exactly why paediatricians care more about the sequence and trend than any single missed date in isolation.

What can parents do?

Keep a simple record of new skills as they appear, even a note on your phone works
Bring specific observations to check-ups rather than a vague "development seems fine"
Offer age-appropriate play, tummy time, reading aloud, simple games, since these genuinely support development
Ask direct questions at each visit: are height and weight tracking consistently, is speech on pace, is anything worth watching

Signs of delayed growth and when not to wait?

A certain amount of variation is completely normal. Some children walk at 10 months, others at 16, and both are fine. What's worth flagging is different from what's worth waiting on.

Reasonable to monitor at home:

A slightly later walking or talking age than a sibling had.
Temporary pickiness with food during a growth phase.
Shyness in new social settings that improves with familiarity.

Needs proper assessment:

Loss of a skill the child previously had, this matters more than a delay.
No words at all by 18 months.
Not walking by 18 months.
Consistent drop-off on the growth chart across multiple visits.
Little to no eye contact or social response by 12 months.
Head circumference is growing unusually fast or slow.

Loss of a previously acquired skill is the one sign that shouldn't wait for the next scheduled visit. It's a stronger indicator of an underlying issue than simply being a few months behind on a milestone.

Catch-Up, referral, and next steps

When a delay is identified, the next step depends on what's actually behind it. A hearing test might explain a speech delay. A closer look at diet and absorption might explain poor weight gain. In some cases, a referral to a developmental paediatrician, speech therapist, or physiotherapist follows, and early referral tends to produce far better outcomes than delaying it in hopes the child simply catches up on their own.

Cost of growth and development evaluation in India

Cost depends on whether the visit involves only a clinical assessment or extends to hearing tests, blood work, or a referral to a specialist.

Factors that affect cost:

The paediatric consultation itself.
Hearing or vision screening, if recommended.
Blood tests to rule out nutritional or metabolic causes.
Referral consultations with a developmental paediatrician or therapist.

When to see a doctor?

Loss of a previously acquired skill.
No words by 18 months, or not walking by the same age.
Consistent fall in growth percentile across visits.
Little to no social response or eye contact by 12 months.
Any concern that persists despite reassurance at home.

Technology at Narayana Health

Narayana Health's paediatric team uses standardised WHO and IAP growth charts to track height, weight, and head circumference over time rather than relying on a single visit's numbers. Developmental screening tools are used alongside routine check-ups to flag speech, motor, or social delays early, and referrals to paediatric specialists are coordinated directly when further evaluation is needed.

Why choose Narayana Health

Paediatricians experienced in distinguishing normal variation from genuine developmental delay.
Access to hearing, vision, and developmental screening under one roof.
Growth tracked consistently across visits, not judged on a single data point.
Direct referral pathways to developmental paediatrics and therapy services when needed.

Conclusion

Most children reach these milestones in their own time, and a few months either side of the expected range rarely mean anything is wrong. What actually deserves attention is a lost skill, a growth curve that keeps dropping, or a child showing no progress at all well past the outer edge of a milestone window. Getting a proper evaluation early, rather than waiting to see if things sort themselves out, is what genuinely changes outcomes when a delay turns out to be real.
