Until a few decades ago, reaching a tumour deep at the base of the brain meant a long incision on the scalp, removing a piece of the skull and gently moving the brain aside. Today, many of the same conditions are treated through the nostrils, with no cut on the face or head. This is possible because of advanced endoscopic skull base surgery, one of the most refined minimally invasive techniques in modern neurosurgery.

What is Endoscopic Skull Base Surgery?

The skull base is the bony floor of the skull. It separates the brain above from the nose, sinuses, eyes, and ears below. It is a crowded and delicate area, because the optic nerves, the pituitary gland, the carotid arteries and most of the cranial nerves pass through small openings in this bone.
Endoscopic skull base surgery uses a thin telescope called an endoscope, fitted with a high-definition camera and a light source. The surgeon passes it through the natural passage of the nostril and reaches the skull base through the nasal cavity and the sinuses. The magnified view appears on a large screen, and fine instruments are worked alongside the endoscope to remove the tumor or repair the defect.
As the nose already forms a natural corridor to the skull base, no external incision is needed and the brain does not have to be lifted or retracted.

Indications of Endoscopic Skull Base Surgery

This advanced procedure is commonly used for:

Complex brain tumours such as pituitary tumors and craniopharyngioma. These often present with headache, blurred or reduced side vision, irregular periods, and unexplained weight change.
CSF rhinorrhoea, the leakage of cerebrospinal fluid through the nose. It appears as clear, watery, salty-tasting fluid dripping from one nostril, often more when the head is bent forward. It needs treatment because it can lead to meningitis.
Skull base fractures, especially fractures of the front of the skull base after road accidents or falls, where the bone and its covering membrane need to be sealed.
Meningiomas of the midline skull base, chordomas, Rathke's cleft cysts, encephalocele and fungal disease of the skull base are also treated through the same route.

What Are the Key Advantages of Endoscopic Skull Base Surgery?

No external scar: The route is through the nostrils, so there is no incision on the scalp or face and no shaving of the head.
No brain retraction: The tumor is approached from below, so healthy brain tissue is not pulled or pressed to create space.
Better visibility: The endoscope goes right up to the tumor and gives a bright, magnified, wide-angle view around corners.
Complete removal: Clear visualisation helps remove more of the tumor while protecting the optic nerves and blood vessels.
Less pain and faster recovery: Most patients walk within a day, and hospital stay is usually two to four days instead of a week or more.
Preserved function: Careful nerve protection helps save vision, hormone function and sense of smell in most patients.

What are the Types of Endoscopic Skull Base Surgery?

The approach is chosen based on where exactly the problem lies.

Endoscopic endonasal transsphenoidal approach: This is the most common type. The pituitary gland is reached through the sphenoid sinus. This procedure is used for pituitary adenomas, Rathke's cleft cysts and many craniopharyngiomas.
Transplanum and trans tuberculum approach: Extends just above the pituitary gland to reach tumors pressing on the optic nerves.
Transcribriform approach: Used for the front of the skull base, close to the olfactory nerves, for olfactory groove tumours and CSF leaks.
Transclival approach: Goes deeper to reach the clivus and the front of the brainstem. This procedure is used for chordomas and some meningiomas.
Transpterygoid approach: A side extension used for CSF leaks in the lateral sphenoid recess and lesions near Meckel's cave.
Endoscopic transorbital approach: A small opening near the eyelid crease reaches the side of the skull base in selected cases.
Endoscopic CSF leak repair: A focused procedure to seal a leak or fracture using the patient's own tissue.

Step-by-Step Procedure for Endoscopic Skull Base Surgery

1. Evaluation and planning: You will need an MRI of the brain with a pituitary protocol, a CT scan of the skull base, hormone blood tests, an eye check with visual field testing and a nasal endoscopy. The team then plans the exact corridor.

2. Preparation: Blood thinners may be stopped on advice, and you will be asked to fast from the night before. Nasal sprays may be prescribed to settle any infection.

3. Anaesthesia and positioning: The surgery is done under general anaesthesia. The head is fixed in position and image-guided navigation is registered, much like a GPS for the skull base.

4. Creating the nasal corridor: The ENT surgeon widens the natural nasal passage and opens the sphenoid sinus.

5. Opening the skull base: A small window is made in the thin bone using fine drills. Doppler and navigation confirm the position of the carotid arteries.

6. Removal of the tumor: Two surgeons work together in a four-hand technique. The tumor is removed in pieces while the pituitary gland, optic nerves and vessels are protected.

7. Reconstruction: The opening is sealed in layers, usually with fat, cartilage and a flap made from the lining of the nose. This step prevents a CSF leak after surgery.

8. Recovery: You are observed in the neuro ICU for a day. The hormone levels, urine output and sodium are checked. Most patients go home in two to four days.

9. Follow-up: Nasal cleaning, a review at two to three weeks, a repeat MRI at about three months and long-term hormones and vision checks.

Case Study

A 50-year-old woman presented with a 6-week history of right-sided watery nasal discharge. Following clinical evaluation and imaging with CT of the paranasal sinuses (PNS) and CT cisternography, she was diagnosed with a cerebrospinal fluid (CSF) leak from the lateral wall of the sphenoid sinus, associated with a meningocele.
The patient and her family were counseled regarding the diagnosis and the need for endoscopic endonasal repair of the CSF leak. The procedure was successfully performed under general anaesthesia.

Postoperative Course

· Nasal packing was placed during the procedure and removed after a few days.

· Both the intraoperative and postoperative periods were uneventful.

· The patient was followed up regularly for three months.

· At the latest follow-up, she was doing well and remained symptom-free, with no further CSF rhinorrhoea.

Frequently Asked Questions

Q. Will I have a scar on my face or head after this surgery?
A. No. The surgery is done through your nostrils, so there is no cut on the face, forehead or scalp, and your hair is not shaved.

Q. How long will I stay in hospital and how soon can I return to work?
A. Most patients stay two to four days. Desk work is usually possible in two to four weeks. Heavy lifting, straining and nose blowing must be avoided for about six weeks.

Q. Is endoscopic skull base surgery safe?
A. It is safe in experienced hands. Possible risks include nasal bleeding, infection, a CSF leak, hormone imbalance and, rarely, injury to nerves or blood vessels. Careful reconstruction and monitoring keep these risks low.

Q. Will I need medicines or radiation after surgery?
A. Some patients need hormone replacement tablets if the pituitary gland is affected. Radiation or medical therapy is advised only when part of the tumour remains, or the tumour is aggressive.

Q. Can the tumor come back?
A. There is a small chance, which is why follow-up MRI scans and hormone tests have been advised for several years. Recurrences found early can often be treated again through the same endoscopic route.
